چکیده
OBJECTIVE To compare the maternal and fetal outcome of 2 different methods of delivering the baby during cesarian section when the fetal head is deeply engaged. METHODS A prospective case control study was carried out in the Obstetrics and Gynecology Department, Al-Thawra General Hospital, Sana'a, Yemen from January to December 2010. A total of 118 women who met our criteria were included in the study. They were divided randomly into 2 groups. A study group (n=59) was assigned to deliver the baby by reverse breech extraction, and control group (n=59) was assigned to deliver by the conventional method. The maternal and neonatal outcomes between the 2 groups were compared. RESULTS Extension of the uterine incision occurred in significantly less women using reverse breech extraction compared to cephalic delivery (5% versus 40.6%; p=0.0001). It was observed that the mean operation time and blood loss in the study group were lower than that in the control group (52.9 +/- 5.1, 787 +/- 519 versus 67.2 +/- 4.7, 1231 +/- 471; p<0.0001). No significant difference between groups was noted in the maternal and neonatal outcomes. CONCLUSION Reverse breech extractions is an attractive and safe alternative to the standard method for intraoperative disengagement of a deeply impacted fetal head in order to reduce maternal and fetal morbidly.

متن کاملDisengagement of the Deeply Engaged Fetal Head during Cesarean Section- Conventional Method versus Reverse Breech Extraction- Review of Literature
Obstructed labor with the fetal head impacted in pelvis is an obstetric complication that requires cesarean delivery with skilful handling. Extraction and delivery of the fetal head in this situation can be achieved utilizing either an ‘abdomino-vaginal’ approach or ‘reverse breech extraction’. Other techniques include use of a ‘Dis impaction system’ or ‘Patwardhan technique’. However, caesarea...

Caesarean section in a severely kyphotic patient presents with unique challenges. We report a case of obstructed labor in case of a pregnant lady with severe kyphosis of spine that was managed by caesarean section. Lateral recumbent position with adequate assistance and paramedian or vertical skin incision was used and found to provide good exposure. Baby was delivered by lower segment uterine ...

OBJECTIVE To compare the morbidity and mortality of 2 current techniques during cesarean delivery of an impacted fetal head. METHODS In a comparative setting, 59 pregnant women with obstructed labor due to impacted fetal head were recruited. The patients were categorized into 2 groups according to method of extraction: the "push" group (n=30) and the "pull" group (n=29). Uterus relaxants were...
متن کاملCommentary: routine cesarean section for breech: the unmeasured cost.
Historically, partial breech extraction under maternal sedation was accompanied by high perinatal mortality rates (1). With the safety of modern cesarean section techniques, many deemed the avoidance of this fetal risk worth the maternal risk of cesarean section, and the proportion of breech fetuses delivered by cesarean section steadily increased. This trend reached its peak after publication ...
